2.5D slipping clutch

Hi

I have just got back from a 800 r mile trip around Scotland. I have noticed my clutch slipping for a while now but on this trip it was slipping quite a lot. It doesn’t  slip in 1st or 2nd gear, 3rd gear not to bad but slipping bad in 4th and 5th gear. Is there any adjustment I could make or is it new clutch time

Ive had the van for around 6 years it’s a 1994 2.5d with 135 000 on the clock. I have been thinking about getting a 3.1td engine for it and have been meaning to contact Dan in pocklington,e yorks .

Appreciate all comments and advice
